Northland Pines Wellness Program Overview Agenda • • • • • PopSpring Overview Your Voluntary Wellness Program Health Coaching Wellness Navigators Questions PopSpring Overview • PopSpring is an independent company based in Central Wisconsin, delivering wellness plan solutions throughout the country. • Locally, PopSpring in partnership with Aspirus, provides onsite biometric services and program support. • The wellness program is available to all employees at Northland Pines. • There is no cost to participate in this voluntary program! Confidentiality and Security • Please be assured that during this process your confidentiality is taken very seriously and will be maintained at all times. • Northland Pines is only able to obtain aggregate, HIPAA compliant reporting information from PopSpring Wellness. • At no time is individual data shared with your employer or your insurance company. Your Voluntary Wellness Program Member Experience Overview Member Experience Overview Receives a WELCOME LETTER introducing the PopSpring Program Sign up for onsite BIOMETRIC SCREENING appointment. Registers on the MEMBER PORTAL and completes the HRA Receives a COMPREHENSIVE WELLNESS REPORT, with a complete picture of their health and contacts a RN Health Coach for a RESULT FOLLOW-UP SESSION. Completes an ONLINE MODULE for cholesterol management which will earn points for the company wellness incentive program. Signs up for a Nutrition CHALLENGE Welcome Mailing – Coming Soon! Mobile App – iPhone, Android • P2Pgo Screenings Wellness Screenings Biometric Testing: 1st Week of January, 2015 Onsite Screenings – Employees are encouraged to participate – Multiple dates and locations are being coordinated for January 5 - 9, 2015 • Testing Includes – Height, Weight, Blood Pressure – Fasting blood draw: Glucose & Lipid Panel • Schedule and how to book your appointment coming soon! Screenings Wellness Screenings Biometric Testing: 1st Week of January, 2015 • You should not eat or drink 12 hours prior to your scheduled testing appointment. • Take prescribed medications • You may drink water and/or decaf coffee prior to testing • Results will be uploaded into the PopSpring portal and included in your 16 page comprehensive member report. – You will be notified via email when the report is ready. – Available after HRA is taken and biometric testing results received. • If you have had a glucose and lipid panel done at your provider’s office on/after 7/1/14 you can submit your own results vs. having another blood draw. Screenings Wellness Screenings Health Risk Assessment (HRA) • HRA will be available to take starting in January 5, 2015. Watch for your welcome mailing explaining how to log on, register and take the HRA. • 10-15 minute online health assessment survey • Used to asses lifestyle, prevention, health awareness, readiness-tochange • Results are confidential! • You can start taking your HRA when you receive your member packet. • The HRA can be taken before completing your biometric testing. • 2 page high level summary available immediately after completing the HRA. • Located on the Member Portal Comprehensive Wellness Report • 16-page report capturing your HRA and biometric results • Available after HRA is completed and biometric data is uploaded – you will be notified via email or text Educational Info Personal Recommendations Health Priorities Wellness Score Telephonic RN Health Coaching Take Advantage of Unlimited Telephonic Coaching! • Talk with a RN Health Coach at anytime! • Schedule a coaching appointment online through your member portal or by call the PopSpring Navigators at 855-699-4445. • Navigators are available M-F, 7:30am-9pm CST • Know your numbers! Schedule your telephonic coaching appointment in February 2015 after your comprehensive report is generated within your member portal. Coaches work with participants to review the member comprehensive report which pulls in their results from their biometric screening and reported HRA responses. • ALL participants are welcome to talk and work with the RN Health Coaches as much as they would like throughout the year! • Employees, spouses and dependents are encouraged to utilize coaching services!
